Section titled “Role Summary”The Medical Forms Coordinator collects and maintains BSA health forms (Annual Health and Medical Records) for all registered Scouts and adults in the pack. Having current health forms available at every event is a BSA safety requirement.

Requirements
Section titled “Requirements”- Registration: Registered adult leader of BSA; current with YPT
- Inherits training requirements from the Secretary role
Core Responsibilities
Section titled “Core Responsibilities”- Collect BSA Annual Health and Medical Records from all registered Scouts and adults at the start of each program year
- Follow up with families who have not submitted forms by the first pack event
- Maintain organized, accessible copies of all health forms (physical binder and/or secure digital copies)
- Bring health forms to every pack event, campout, and outing
- Ensure health forms are updated annually or when a Scout’s medical situation changes
- Coordinate with the Secretary on BSA Activity Consent forms
- Remind families before campouts and special events to update forms if needed
- Securely store and handle all medical information in compliance with BSA privacy policies
